Meeting Criteria

GSP’s team of highly trained and skilled medical case managers delivers a comprehensive suite of client-centered services designed to facilitate access to the quality clinical and supportive services they need to achieve and maintain optimal health and viral suppression. GSP’s case managers are located at our main office as well as at several satellite locations. Our team is happy to meet you in the community or at your medical appointment. We offer bilingual (Spanish/English) case management services as well. Clients eligible for case management services meet the following criteria:

Have an HIV/AIDS diagnosis

Have an income at or below 300% of the Federal Poverty Level.

Live in the 11-county area (Missouri: Cass, Clay, Clinton, Jackson, Lafayette, Platte, and Ray; Kansas: Johnson, Miami, Leavenworth and Wyandotte) or receive primary care within the 11-county area.

Basic Steps

If you’re interested in obtaining case management services, or if you’ve lost contact with your case manager, please contact GSP at 816-561-8784 and request to speak with the case manager of the day. The steps for obtaining services are below:

  • Complete a brief intake and screening form so that GSP can assess your need for services and determine the best location for case management – whether that’s GSP’s main office, one of our satellite sites or co-located at one of the HIV clinics or hospitals.
  • The intake and screening form is handed off to the Director of Case Management Services for assignment.
  • Your assigned case manager will arrange for enrollment appointment and complete your intake assessment. You’ll work together to develop an Individualized Service Plan (ISP) to address your needs and overcome any barriers to care.
  • In order to expedite the process of enrolling in case management, please bring proof of your HIV status. We will also need documented proof of residency, income and insurance.
  • Our clients are referred to the appropriate services based on the needs identified during intake and enrollment. Our goal is to ensure our clients have access to medical care and are continuing their medical care treatment – but we recognize that this is not always the client’s top priority. Our case managers work with clients to overcome any number of health care barriers including housing, substance abuse, mental health issues and more. Our goal is to help our clients be successful in their medical care.
  • Once enrolled, clients must re-verify eligibility every six (6) months and speak with their case manager to provide an update on their medical care.

Our primary case management goal is ensuring that all clients are linked to and remain regularly engaged in medical care, an essential element of maintaining optimal health. GSP case managers work with clients to facilitate access to medical care. Our case managers and clients are partners – and that partnership is most successful when both are active participants.

Clients who participate in case management may be eligible for the following programs through Ryan White:

AIDS Drug Assistance Program (ADAP) in both Missouri and Kansas assists with full cost of medication for uninsured clients. For clients covered by private insurance, ADAP helps to offset out-of-pocket drug costs, such as co-pays or co-insurance. ADAP may also assist with Medicare- or Medicaid-related out-of-pocket costs.

Health Insurance Continuation Program (HICP) assists with private health insurance premiums, co-pays and deductibles. This includes private plans purchased through the Affordable Care Act's (ACA) HealthCare Marketplace (also known as Obamacare).

Transportation Assistance is available in the form of bus passes or gas cards for medical appointments.

Transitional or Permanent Housing Assistance Note: This assistance is extremely limited in our region.

Mental Health Counseling by licensed therapists. GSP offers a variety of therapists and counseling options. This service is available for no cost for those Ryan White clients who are uninsured. Clients with private health insurance may receive assistance in finding a therapist that accepts your insurance.

In addition to medical case managers, GSP houses a transitional case manager – one of only three in Missouri – who is specially trained to work with clients who are currently or have recently been incarcerated. Our transitional case manager functions as a medical case manager but has a specialized focus on the incarcerated and the unique challenges that comes with a history of incarceration. The transitional case manager has access to additional service referrals for these clients.

GSP also has a client services coordinator who works with case managers and clients to provide limited emergency assistance, limited bus passes, and access to the GoodStore for hygiene products for clients enrolled through GSP. Services are dependent on funding and not guaranteed.

Our client services coordinator in partnership with our case managers also helps link eligible clients to our 340B pharmacy, Coordinated Care Network. You may be eligible to utilize our pharmacy if you have Medicare or private insurance.
